FirstImpact Med respects each patient’s right to safe, respectful, informed, and private care during intake, physician review, documentation, billing, and authorized record access.
Your Rights as a Patient
Right to Respectful Care
You have the right to be treated with dignity, courtesy, and respect without discrimination.
Right to Clear Information
You have the right to receive information about your evaluation, care process, clinical recommendations, limitations of asynchronous review, and the roles of the professionals involved in your care.
Right to Informed Consent
You have the right to understand the nature of FirstImpact Med’s services before you consent, including telehealth intake, physician review, documentation, privacy practices, billing pathway, and any authorized representative access.
Right to Refuse or Stop Services
You may refuse or stop services to the extent permitted by law. If you refuse recommended care, escalation, or emergency guidance, you may be informed of possible medical risks.
Right to Privacy
You have the right to privacy and confidentiality regarding your medical information. FirstImpact Med handles protected health information according to privacy and security safeguards.
Right to Access Records
You have the right to request access to your medical records and to request correction of information you believe is inaccurate or incomplete.
Right to Billing Transparency
You have the right to receive information about charges, billing pathway, lien-related documentation when applicable, and available payment information before services are finalized.
Right to Control Authorized Access
You have the right to decide whether an authorized representative may access your case information, subject to your signed authorization and applicable law.
Right to Ask Questions or File a Complaint
You have the right to ask questions, raise concerns, or file a complaint without retaliation.
Your Responsibilities as a Patient
To help us provide safe and accurate care, we ask that you:
- Provide accurate and complete information about your accident, symptoms, medical history, medications, allergies, prior injuries, and treatment since the accident
- Answer safety-screening questions truthfully
- Tell us if symptoms worsen or if new warning signs appear
- Call 911 or go to the emergency room for emergency symptoms
- Ask questions when you do not understand the process, recommendations, forms, or billing pathway
- Use the secure portal when available for sensitive health information
- Treat FirstImpact Med staff and clinical team members with respect
- Review forms carefully before signing
- Fulfill agreed financial or lien-related obligations when applicable
Filing a Complaint
If you believe your rights have been violated or you have concerns about your care, documentation, billing, privacy, or access to records, you may contact FirstImpact Med at info@firstimpactmed.com.
Complaints are reviewed without retaliation. If your concern involves privacy or protected health information, you may also have the right to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
