You have the right to receive information necessary to give informed consent before any procedure or treatment begins. This includes information about risks, benefits, and alternatives.

Right to Refuse Treatment

You have the right to refuse treatment to the extent permitted by law. You will be informed of the medical consequences of your decision.

Right to Privacy

You have the right to privacy regarding your medical care and personal health information. All communications and records related to your care are treated confidentially.

Right to Access Records

You have the right to access your medical records and to request amendments to your records if you believe they are inaccurate or incomplete.

Right to Continuity of Care

You have the right to know in advance what appointment times and physicians are available and where. You have the right to expect reasonable continuity of care.

Right to Be Informed of Charges

You have the right to receive an explanation of your bill regardless of the source of payment and to receive information about financial assistance if needed.

To help us provide the best possible care, we ask that you:

  • Ask questions when you do not understand information or instructions
  • Follow the treatment plan recommended by your physician
  • Accept responsibility for the outcomes if you refuse treatment
  • Treat our staff with respect and courtesy
  • Fulfill your financial obligations in a timely manner

Filing a Complaint
